All our PCN staff work closely and compliment practice teams to help support and build a broader offer of health and care to our patients and also work closely with other services in the local community to provide a more integrated service.
Leadership Team
The PCN Leadership team consists of staff who help to run and manage the network on a weekly basis. Their roles are varied with some working in practices and some working alongside other organisations but ultimately all contribute towards the smooth running of the PCN.
Every month the team meets along with team leaders, partners and practice management teams to plan the future and discuss current challenges within the network..

Link Workers are non-medical support staff who focus on a ‘What matters to me conversation’ with people, they help to create a plan together and provide an holistic approach to health and wellbeing. Connecting people to local services, resources and community groups, they find what suits their needs and help with practical and emotional support.
Health Coaches support people to identify how they can implement sustainable health and lifestyle changes in a way that is meaningful to them and benefits their health.

Pharmacy Team
The pharmacy team consists of prescribers, or completing training to become prescribers and work alongside the practice team . They help with care management of patients with chronic diseases and do clinical medication reviews to proactively manage people with complex polypharmacy. Pharmacists have specialist expertise in the use of medicines to help address both the public health and social care needs of patients at practices and to help in tackling inequalities. They lead on person-centred medicines optimisation and quality improvement.
Through structured medication reviews, support patients to take medications to get the best from them, reduce waste and promote selfcare.

Physiotherapists
Our PCN physios are triage based physios helping to guide patients on the best available options for their musculoskeletal issues, they predominantly offer face to face appointments but also have capacity for telephone appointments.
They provide patients with advice, education, home exercises, possible lifestyle changes and signpost to relevant services or information to help patients manage their musculoskeletal problems effectively. If required physio’s can arrange certain investigations for patients where appropriate.

GP Assistants
The GP assistant team consists of 5 staff members with a whole range of skills including phlebotomy, HCA, reception and administrative skills helping the practices to utilise them where is needed most. The other side of their work is contributing to the network projects becoming champions in their fields to better help educate patients on the possibilities available to them within the local community and healthcare setup.

Primary Care Mental Health Team
The primary care mental health team (PCMHT) consists of many different roles including therapists, practitioners, psychologists, pharmacists and administrative staff. They all help to handle and support complex/severe mental illness cases. They take a person-centred approach, helping patients to improve their mental health, and supporting them through treatment or short-term interventions.
Mental Health Practitioner
Assessment appointments, formulating care plans, delivering appropriate interventions depending on patient’s needs, supporting with onward referrals to other services.
Senior Cognitive Behavioural Therapist
Therapeutic work including brief Cognitive Behavioural Therapy (CBT), short-term formulation work and medium length Trauma-focused Psychotherapy.
Clinical Associate Psychologist
Short-term interventions which focus on formulation, psychoeducation, and skills development on a one-to-one basis and in a group environment.
Community Connector
Up to 10 sessions of 1:1 support, enabling patients to build the confidence to access services/groups within their local community. Overcoming anxiety and building self-esteem, understanding coping mechanisms, and engaging in directed work towards an agreed goal.
Mental Health Wellbeing Practitioner
Short-term treatments focusing on building skills to help manage low mood, anxiety, sleep problems, low self-confidence, difficulties managing emotions; developing skills in problem-solving.
